Original 1962 Evansville, Indiana Employment Certificate issued to 16-year-old James Michael Cole. This Form G work permit documents age, schooling, prospective employment, parent or guardian information, employer, occupation, and issuing officer William C. Jardine of the Evansville School Corporation.
